BURLINGTON YOUTH LACROSSE, INC. WAIVER AND RELEASE OF LIABILITY

By registering a participant for any Burlington Youth Lacrosse, Inc. program, the parent/guardian acknowledges and agrees to the following:

Assumption of Risk

I understand that lacrosse is a contact sport and that participation involves inherent risks, including but not limited to sprains, fractures, concussions, and other injuries or even death. I voluntarily assume all risks associated with the participant's involvement in Burlington Youth Lacrosse programs, including practices, games, clinics, and related activities.

Release of Liability

I, as the parent/guardian, on behalf of myself, the participant, and our heirs, release and hold harmless Burlington Youth Lacrosse, Inc., its officers, board members, coaches, volunteers, and agents from any and all claims, demands, or causes of action arising from the participant's involvement, except in cases of gross negligence or willful misconduct.

Medical Authorization

In the event of an emergency, I authorize Burlington Youth Lacrosse personnel to seek and consent to medical treatment for the participant if I cannot be reached.

Photo and Media Release

I grant Burlington Youth Lacrosse, Inc. permission to use photographs, video, or likeness of the participant for promotional purposes, including the website and social media.

US Lacrosse Membership

I understand that all participants on our Spring Season Teams are required to be registered members of US Lacrosse and that it is the parent/guardian's responsibility to complete this registration separately.

Equipment

I understand it is the parent/guardian's responsibility to ensure the participant has proper, well-fitting protective equipment for all practices and games.

By completing registration, I confirm that I have read, understand, and agree to this Waiver and Release of Liability.
